Terms and Consent

These are the patient-facing terms for the RESET program. Clinics can later replace this text with their own policy set.

Program use

RESET is a clinician-supervised weight-management program. It is not an emergency service and it does not replace medical care.

Privacy and records

Information entered into the program is used for care delivery, clinical review, follow-up, reporting, and record keeping. The clinic may keep program records in accordance with applicable retention rules and professional obligations.

Communication consent

You agree that the clinic may contact you by email, secure portal, or other program communication channels for registration, reminders, follow-up, and approved documents.

Patient responsibilities

You agree to provide accurate information, update the clinic when important information changes, and contact your care team if symptoms or concerns change.

Clinical judgment

Reports, meal plans, and recommendations are intended to support clinician judgment. Final care decisions remain with the clinical team.
