When you register for an account or use our website, patient portal, or mobile application, we collect the information you provide to deliver our services, create and manage your account, communicate with you, coordinate your medical care, process appointments and payments, and improve our services.

Depending on the services you use, we may collect personal information such as your name, date of birth, contact details, health card information, emergency contact information, medical history, medications, allergies, laboratory results, body measurements, consultation notes, and other information necessary to provide safe and effective healthcare.

Your personal health information is collected, used, stored, and disclosed in accordance with applicable Canadian privacy laws, including Ontario's Personal Health Information Protection Act (PHIPA). We do not sell your personal information.

Your information may be shared only with healthcare professionals involved in your care, laboratories, pharmacies, specialists, hospitals, service providers that support our operations, or where required or permitted by law. All service providers are required to maintain appropriate confidentiality and security safeguards.

RESET may use Artificial Intelligence (AI) tools to assist with documentation, generating medical notes, drafting referral letters, preparing patient education materials, and improving administrative efficiency. AI is used only as a clinical support tool. All diagnoses, treatment decisions, prescriptions, referrals, and medical records are reviewed and approved by a licensed healthcare professional before becoming part of your care.

We use industry-standard administrative, technical, and physical safeguards to protect your information, including encryption, secure cloud storage, role-based access controls, audit logging, and secure authentication. While no electronic system can guarantee absolute security, we take reasonable steps to protect your information from unauthorized access, use, or disclosure.

By creating an account and selecting "I Agree," you acknowledge that you have read and understood this Privacy Notice, consent to the collection, use, and disclosure of your personal and health information as described, and agree to our Terms and Conditions and Privacy Policy.

You may withdraw your consent for certain uses of your information where permitted by law; however, doing so may limit our ability to provide healthcare services.
